During your study at Massey you may also need to fill in some of the following forms.

Supplementary forms

Supplementary forms include the following forms:

• application for credit towards a Massey University qualification
• application for waiver of paper prerequisite or corequisite
• registration form for disability service
• additional assistance in exams form
• distance learning and block exam venue registration form
• payment of fees form 
• change of exam venue - for distance and block courses form.

Administrative forms

Administrative forms include following forms:

• request for student ID card form
• fee appeal form
• agent authorisation form
