The multidimensional aspects of managing long term conditions within contemporary society, including self-management and models of chronic care, are examined and applied to improve health service delivery, and optimise health and independence.

  • 4 Use chronic care models to design and justify a service delivery model for use in a clinical setting which improves the health outcomes and quality of life for people with long term conditions.
  • 5 Integrate interdisciplinary team and collaborative approaches, which include partnership with the individual, whanau and community into the management of people with long term conditions.
