- application for credit towards a Massey University qualification
- application for waiver of paper prerequisite or corequisite
- registration form for disability service
- additional assistance in exams form
- distance learning and block exam venue registration form
- payment of fees form
- change of exam venue - for distance and block courses form.
Administrative forms
