Referrals to the Cancer Psychology Service

You can be referred to the Cancer Psychology Service by any health professional. This includes your:

  • GP
  • Nurse
  • Māori Cancer Coordinator
  • Specialist
  • Māori health provider
  • Other health professional

To be referred, talk to your health professional about coming to see us. He or she will fill out a referral form for you and you will need to fill out a self-distress form. You can give your completed self-distress form to your health professional to send to us with the referral.

Health professionals – please download the

CPS Referral Form (54 KB) for your patient to be referred and the

CPS self distress form (37 KB) for your patient to complete (and give back to you, the referrer).

What happens next

We will let your referrer know that we have received your referral and will determine whether our service is appropriate for you. If your referrer doesn't hear from us within 24 hours, they can contact us.

We'll ring or write to you to set up an appointment. When you come in we'll ask you about how things are. The first appointment will be for you to get to know us, and for us to get to know you and talk about what's happening for you. After this, we'll set some goals for what you want to get out of the sessions and we'll start working towards achieving them.
